How to Prepare for Your Appointment

Before your upcoming appointment, you will receive an email from MyChart_eCheckin@sentara.com with a link to complete the new patient registration process, including a health questionnaire and new patient forms. In addition, you will also be able to pay your co-pay online. To save time, we ask that you complete your registration online before arriving at your appointment.

Day of Your Appointment Instructions

If you completed your patient registration online and no additional information is needed, you will be sent an email 30 minutes before your scheduled appointment time allowing you to virtually check-in and let our care team know you have arrived for your appointment. Our team will then greet you and screen you for COVID-19 symptoms as you enter the practice. Please bring your insurance card, co-pay (if not paid in advance), a valid driver’s license or ID and a list of any medications you are currently taking to your appointment.

If you choose not to pre-register online, please arrive to your appointment 15 minutes in advance to complete your patient registration forms.

Please note that if you are unable to show up at your scheduled appointment time, we kindly ask that you cancel or reschedule your appointment. To cancel or reschedule your appointment, please call our Care Connection Team at 1-844-615-1237. A $40 fee may be charged to you for any appointment missed or cancelled less than 24 hours prior to your scheduled appointment time.

Text Messages From Your Care Team

In addition to a phone call from your provider’s team, you may also receive a text message from them on your mobile phone to update you on your appointment details or share important information about their office or your provider. Text messages from your Sentara Medical Group care team will come from #50771. You will not be able to reply to these messages, so please call the practice directly if you have a question or concern.
